<p>I am a permanent resident and I need to send a copy of my permanent resident card to UConn. I don't know how to indicate that it's me who is sending the card. Do I write a letter or include a piece of paper with my name on it? I already applied EA, but still haven't send my permanent resident card.

<p>When I dealt with UCONN & I faxed or mailed them anything, every piece of paper had my child’s name, address, phone # & Social Security Number on it, because it is such a huge school.</p>
<p>I would recommend mailing a copy, or faxing a copy, with a cover letter. I would not recommend putting your full SS number on it, but perhaps the last four digits.</p>
<p>Thanks for the responses. I actually contacted UConn and they told me to include my full name, DOB, address and phone number. I put everything on one sheet of paper.</p>
