<p>The forms are very similar, so it's not very difficult to have your advisors/teachers fill out the ND forms, but yea, I would definitely send the ND forms, they made them for a reason.</p>
<p>As far as "too late," I'm pretty sure my letter of recommendation didn't get there for like another month, with no reprecussions. You should get a letter sometime soon saying you're missing xyz form, could you please send it in. </p>
