Sending teachers' evaluations and transcript

<p>i'm not sure as to how i should send teachers' evalutaions and the school transcript.
do i just send them in one envelop addressed to certain colleges? or
is there any special form that i must follow for each school sending those documents?</p>

<p>also, are those two the only documents each college requires?
i'm not sure if there's anything other than those two which the colleges would
want to see.(maybe mid-year report, which should come later)</p>

<p>Schools are different. Carefully observe what they required. For example, UC just needs your transcript but no evalutions form. And certain college has certain information which is hung on the website</p>
