SHIP wavier

<p>I try to fill out the SHIP wavier. My mom is an OUSD employee, and we have membership in Kasier Permanente. I have some questions,
What is Policy Holder's Name?
What is Policy Holder's Identification Number?
For most of the service, there is a copayment. ex. $100 per visit for Rmergency Department. (not by cover %) I am not sure if my plan fulfill all requirement, should I just click on yes?</p>

<p>I have Kaiser too.. I was wondering if it'd cover the requirements that SHIP has.</p>

<p>We have Kaiser as well and waive SHIP for son. I believe the policy holders name would be your Mom since you have it through her employment. And she should be able to get the policy ID number through work.</p>

<p>Last year I remember having to call up my insurance company to find out if my son could waive SHIP. I went line by line regarding each question, so could answer "yes" to them all. We have Blue Cross, though, not Kaiser.</p>

<p>I doubt anyone at TANG has the answer -- in fact, when you go online to opt out, they put you through the wringer, they are so anxious for you NOT to opt out -- trying to find the link to opt out is an intentional ordeal, if you ask me.</p>

<p>BUT, $1,000/year is $1,000 more I have to spend on my son for other things, like a mini-vacation or a few rock concerts (yes, I am an ardent subscriber to the ethic "work hard play hard"); and our insurance is more than adequate.</p>

<p>I have Kaiser and I just busted out my Kaiser ID card to waive SHIP. My dad got so angry when I asked him if we could waive it because they hide the link and he found it ridiculous that Cal mandated insurance. </p>

<p>The policy ID number was my MRN (on the Kaiser card) and the policy holder is me because it's in my name (on the Kaiser card) although it's on my mother's insurance. I just hit yes to everything and although it's only a $1,000, again it's a thousand or so that I didn't need to pay, you know?</p>

<p>Are there any incoming sophomore/junior/seniors that went through this experience?</p>

<p>You do it more than once, I'm told by my friend who is now a senior.</p>
