Should I address ADHD in application?

<p>I am a 26 year old community college transfer student. In the beginning my grades, along with my focus, were a mixed bag. In the first few semesters my grades alternated between decent (A&B) and horrid (D’s and W’s). I decided to submit myself for a psycho-educational evaluation, because my issues with focus were becoming progressively worse and nearly insurmountable over time. I was diagnosed with ADHD.</p>

<p>Since the diagnosis I’ve retaken earlier classes and my grades steadily improved. -First remaining stable in the A/B range and later moving into the straight A category. As a result, my gpa is now a 3.5. My extracurriculars have remained good through out my the struggle with my issue.</p>

<p>Do you suggest I address my issue and what I have done to resolve it my personal statement? Should I provide info as a supplement? (Yes, I am aware that transfer spots can be described as minuscule at best.)</p>
