I got an email from Berkeley asking me to fill in an Augmented Review questionnaire today and it looks like it's the disability/medical issues one. I mentioned something in the additional info on my regular app to help explain my B's in math/science, but it's not really a serious disability and it doesn't really affect my life that much now.

If I fill it in, I think I would kind of have to make some stuff up. Should I still fill it in?

Stats-

in state

And I applied to Letters and Sciences.

Think you answered your own question there when you said “have to make some stuff up.”

However, in telling the truth you still have all those blank spaces to sell yourself. Thus, explain it was a temporary situation that impacted you, but in spite of it you were able to get at least a B and have learned how to overcome challenges, deal with distractions, etc.

Then add in anything that you would have said about yourself if you were talking to the admissions reader and they asked - tell me what, besides the information you listed so far on the application, should make us choose you instead of someone else for a spot at Cal. You also can expand on anything good, tell more about ECs, life experiences, whatever might make you more appealing or unique or stand out.

Plus you get to submit a reference and first half grades, something that more than 95% of applicants cannot do.
