<p>Yes, I am stupid, I just now remembered that little checklist we got a while ago in the mail about forms we needed to complete, and I I did all the other required ones, except the health forms now. </p>
<p>It says they were due June 14. I should be able to get the online stuff submitted by tomorrow or the day after by getting the immunization forms, but then there is a problem with the physical. I may not be able to get a physical relatively soon, so two questions:</p>
<p>If I already had a physical within 18 months, do I have to get another, or can my doctor just fill in the info for that one? If I need a new physical, how bad will it be if I get those forms in late, possibly as late as July depending how booked up things are?</p>
<p>I think I saw something like it doesn’t have to be in until around August (don’t trust me on this) but there will definitely be a late fee/penalty.</p>
<p>My doctor’s office won’t fill out a form if they haven’t had a checkup in 12 months. July should be fine. You just need to have the health form before you could participate in any sports, and maybe before you could attend the school. I think what’s more important is the waiver for health insurance. If you don’t have proof of insurance, then Cornell would bill you for the school’s health insurance.</p>
<p>Yeah I think you can send it in july/august before the semester starts but there is a penalty fee. (Think $100 or something, it’s on the health services site).</p>
<p>All new students (Undergraduate, Transfer, Grad, Vet, Law, JGSM) must return a completed health history form, demonstrating compliance with immunization requirements by the following deadlines. </p>

<p>Upon receipt of your form, Gannett Health Services will review it to make sure you have met all requirements. If you have not achieved full compliance with all requirements by August 14, 2009 (January 15, 2010 for spring entrants), you will not be registered and you will be charged a $100 late fee.</p>
