Signature and date printing in common app teacher evaluation form--get rid of it how?

<p>Need to resend one teacher's evaluation form, and the high school only does paper apps. So I click on the pdf thing to print the form, but the student signature line (in the waiver block) and a date (not the correct date, a date from December) show up. How can I delete these so a real signature and the correct date can be written in??</p>
