yes, you have to apply for a sponsor family.
yes, you can request a specific family as long as they have been through the training and live within a 22 mile radius of the Chapel dome.
if i remember correctly, the paperwork is in the permit to report package, but not due immediately. more likely due by I-Day.

Ok, I don't know a lot, but maybe I can help and others will chime in to correct me or add to.

If memory serves me right, in last years <disclaimer> PTR packet among a ton of paperwork, there was a web address to a questionnaire regarding the Sponsor program. It's sort of like a 'matching' questionnaire.</disclaimer>

I honestly don't know if you can request a family. However, I do believe a family can request you.

I am a Mid Parent and Sponsor. Yes, you can request a specific sponsor but both the sponsor and the Mid must request each other. At some point over Plebe Summer the Mids are given a form to fill out if they would like a sponsor and if they have a special request - such as a specific sponsor or to have the same sponsor as another Mid -again both Mids must have the same request. The Sponsor -if a first time Sponsor must fill out a back ground form and be approved - then they must take a class at the Academy. On the forms that the Sponsor and Mids fill out they do try to match requests but they don't always get it correct. We got a Mid that we did not request but it all worked out - That Mid became great friends with us and all of his sponsor brothers.

- not every Mid has a Sponsor....some Mids by choice and others because they do not have enough Sponsors.
the sponsor paper work should be in your report packet but I know for the last few years that they have the Plebes fill it out again over the summer.
another question often asked is do you get paid to be a Sponsor? - That answer is no. I think it costs us about $400+ a month in extra food for the Mids for weekends.
Not all Mids and Sponsors click. We have been very happy with all the Mids that come to our home. We have 5 sponsor Mids (they do come almost every weekend) but any given Saturday or Sunday we could have from 3-10 Mids come to our home. We get extra Mids often.
-Our Mids like to come to our house and chill-out, watch TV, sleep, play sports outside, oh and EAT.....ect.......
-It is a lot of work to be a Sponsor but it is really fun, too. We have formed wonderful friendships with our sponsor Mids.

Sponsor Parents are a plebe's sanctuary. As a youngster, I've found myself abandoning my sponsor parents a little bit more than last year, but my sponsor dad is a grad, so we always manage to keep in touch. Trust me, fill out the paper work and get a sponsor family. Even if it doesn't work out, they volunteer for it, so they'll be there for you when you need them.

My story is funny...my 10th grade history teacher's sister is my sponsor... they requested ME before I even got there and it was suprise to me that this had somehow been "arranged". I just filled out the preferences, but I still ended up with them. It's really a great program and highly encouraged... and they have three daughters so it's not that bad.

It's a questionnaire you get online before plebe summer. It asks about smoking and pets and stuff like that. It generally sucks to be a sponsors only mid though. Or a first time sponsors mid because they'll be weird about letting people come over. And it gets boring being alone. That's why I requested a new sponsor and I love them and my new sponsor siblings.
You can put it in the comment section about having a sponsor with daughters but they'll probably ignore it and just think you're a hornball. There area lot of stories of mids marrying their sponsor sisters though. So you may get lucky.

Just to let everyone know, I was almost completely kidding about the whole daughters thing, lol. But seriously, are you able to go to your sponsor's house anytime you have town liberty?
