<p>For Stanford's Optional Evaluation for transfer applicants, what forms are required? Should I just ask for a letter, or is a form required for them to be filled out?</p>

6. What about additional evaluators?</p>

<p>You may submit one additional letter of evaluation if you have another person, not a teacher or counselor, who knows you well and will provide us with information about you that your other recommenders will not. We ask that you instruct your additional recommender to place your full name–as listed on your application, your birth date, your Common Application ID number, and your current high school at the top of the recommendation letter. You can secure your Common Application ID number from the other forms you download from the Common Application website after logging into your account. Unlike the mailing labels we provide for Teacher Evaluations, there is no special label or form that you should use in the mailing of this optional letter.
