Stanford's Optional Evaluation

You may submit (a maximum of) one Optional Evaluation if there is
another person, not a teacher or counselor, who knows you well and
will provide us with different information and new insights about you.
We ask that you instruct your additional recommender to place your full
name (as listed on your application), your birth date, and your current
school at the top of the letter. This is entirely optional and you are not
expected to submit this additional evaluation." </p>

<p>Which form is that on CommonApp? Is it the Secondary School Counselor?</p>

<p>From the instructions you cited, they say to have your name, etc. a the “top of the letter”, which leads me to think that the recommender can just send it on their work/company/etc. letterhead and don’t need a form. If you want to use a form, the CA one for Instructor LORs would be more appropriate than the SSC.</p>
