Do you put it in any kind of binder or presentation folder? Do you just paper-clip it together and stick it in the envelope?

Paper-clip or staple it together and put it in an envelope, no special presentation required or desired. (But do remember to put sufficient postage on the envelope! It will probably weigh more than a normal letter; my kids' school has told them to put 3 stamps on the envelopes.) The Admissions office will unload it into whatever folder they use to keep track of each applicant's stuff (and will add recommendation letters, transcript, etc. as they arrive).

Second mootmom's suggestions. Remember to put applicant's name and DOB or some other form of ID onto every single page. Files get dropped or mixed up and it's easier to reconstruct them if all the sheets bear ids.
