SUNY chances with not great GPA and severe LD

Daughter has severe dyslexia and dyscalculia. 9th grade she did great except for math which she barely passed (with lots of help).

She did so good last year (in all other courses) that for 10 grade, teachers and all recommended AP World, H English. She then cut out lunch and resource room took two sciences and creative writing. She is bombing right now she has mid 70's in most, 67 in math. 100 in both creative writing and Photography.

We have made changes for next semester and next year already. My question:
She desprately wants to get into a college. It will have to be SUNY for financial reasons. Her major would be in Art. Her top choices are New Paltz and Plattsburg which I think are very competitive.
What are chances getting into these schools if she brings her GPA up a bit?

She volunteers everyweek at camp for severely disabled and is in many after school clubs including Art Honors Soc. Stage crew, Newpaper, yearbook, etc.
