Super Duper Important Midterm Question

For schools that don't require midterm grades but will take them... what do you do for creating forms. I'm planning on including course #, course title, prof's name, prof's e-mail, and then a place for the prof to write the grade and the prof's signature.

Anways... I think I'm just going to go letter grades but does anyone think I should have percentages or perhaps both.

I got two A's (one is a 100, the other I'm not sure), two A-'s (one I believe is a 91; the other I'm not sure), and one B+ (not sure percentage). Obviously I'd ask the prof's for the percentages.

The grades were a little lower than I had hoped for... works out to a 3.71 when you differentiate between 3 and 4 credit classes... but I had a 3.46 last semester and this is much closer to the 3.81 and 3.74 that I received in my first two semesters. It also corresponds better with the 4.0 u/w I had in high school. Hopefully this shows schools that last semester was a fluke.

Anyways... I guess I just want to know about grade vs. percentage vs. both being included. Leaning towards grade at the moment.
