Supp. rec. identifying info?

<p>So, someone is writing a supplemental rec for my app to some schools. Now, I imagine they need all my personal information included somewhere on that letter so they can connect it to the rest of my app. On the common teacher rec form, this information would be included in the "to the applicant" section. Should I give the writer a sheet with that same info so it can be stapled to the letter? What is the best way to do this?</p>
