Supplemental Recs

I am going to ask some people (such as my mentor) to fill out supplemental recs for MIT. Now that I actually have to do it, I'm not sure how it works... Should I just tell them to write a general letter and stick it in the envelope and send it to the admissions office? Or should I give them the MIT evaluation form? If I just get them to write a letter and send it alone, how will the office know clearly who the evaluator is evaluating on behalf of? If I use the MIT evaluation form, they might think it is one of my two main recommendations?

Any ideas, suggestions? Thanks.

NVM : I found the answer in another thread.

Thanks anyway. If you have anything to add, feel free.

Here is the response, in case anyone was looking for it:

I have two additional rec letters. Both are informal and do not need to follow the teacher rec guidelines. They are very flexible and the writing style can be anything as long as it paints a more holistic picture of you.

You just send it via mail and make sure to include your name and birthday so they can match it up with your folder.
