<p>I'm having a retired teacher who worked with me a lot in my junior year for a competition write me a supplemental rec. Does she have to fill out the Common App Teacher Evaluation form or can she just write it as a letter? Also, if I have it mailed to my schools, how does the school know that it's me? What information other than name do I have to attach so that the supplemental rec would be linked to my application?</p>
<p>If the school you’re applying to is a Commonapp school, then i would ask the teacher to fill out the COmmon app form. If it’s a school with its own separate application, then the letter would be fine (although for some schools, there is a teacher recommendation form you might need to attach with the letter). </p>
<p>As for what other info to send with them, I put my birth date. Some online applications also have an applicant ID, so I included that too if there was one.</p>
<p>My daughter has a supplementary rec. Only half her schools would allow for this supplementary rec to be uploaded with her common app. So, to keep things simple, we gave her rec some stamped and addressed envelopes. We asked her to put my daughters CA number in the letter. </p>
<p>She does not need to fill out the form.</p>
<p>The Common App is for academic recommenders. Additional recs should be sent separately as a letter (no form).</p>
