Supplementary Materials?

<p>I want to send a clip on a dvd, but I don't know how I should send it? Should I write my name and SS number on the disc, send a piece of paper with my name and SS number, or provide an introductory description with my name and SS number? Thanks.</p>

<p>I don't think they would look at a dvd. Some colleges have their own forms for supplementary material.</p>

<p>First call admissions (important!) and make sure they will accept a video. Then ask if sending a DVD is OK or if it has to be a VCR cassette.</p>

<p>Then label it with your name, birthdate, and SS number--on the actual disk or the disk case.</p>
