<p>They don't need any forms or anything, right? Just a letter from them is fine, correct?</p>
<p>I worked with a Physics Professor this summer one-on-one doing research. He said he will absolutely write a college rec for me, and just wants to know the details.</p>
<p>No form, right? And for the rec, I mean, he can pretty much do what he wants with it right, putting whatever he wants to say and making it whatever length?</p>
<p>I just had my GC put it right in the rest with the other recommendations, and then I stated on the cover letter that a supplementary recommendation from my superintendent was included.</p>
<p>I did <em>not</em> however, include anything about SS or DOB. Uh oh.</p>
<p>You don't have a problem if the supplementary rec is being sent by the guidance counselor with your other official materials. A piano teacher or an employer, for example, who is sending a letter directly to the Admissions office, should include this info so that his/her letter is sure to go in the correct student's file.</p>
<p>I think I would just rather him send it himself. Where should he put this info with the rec. I think it would seem awkward if he said "It is my pleasure to recommend Matthew, born on blah blah, with social security xxx-xx-xxxx...."</p>
