<p>So I very recently found out that my teacher did not fill out the evaluation form provided by the Common Application and only included a letter of recommendation in the sealed envelopes that I just got finished mailing this morning!</p>
<p>And to make matters worse, she moved across the country at the end of this semester and I have no contact information for her beyond an e-mail address that I'm not even sure she checks anymore. What do I do?</p>
<p>As far as I know, nothing is wrong. Teachers at my school do not fill out the eval forms; they staple their letters to the eval forms and send them in (i.e., the eval forms are blank and just used to sort the recommendations).</p>
<p>One of my teachers didn't fill out the forms, and they weren't included in the envelope either. I sent it EA to Caltech, and they counted it. I was deferred not rejected, so obviously it's not the end of the world.</p>
