Teacher Evaluation Question

So I printed one of those teacher evaluation forms from the Common App website and gave it to a teacher. Is the evaluation the same thing as the recommendation? In other words, is the teacher supposed to include a recommendation with the evaluation, or is the evaluation enough by itself? Sorry if this is a dumb question, but I'm kind of confused right now and I need to tell my teacher whether or not I need him to type a letter of rec.

Also, I may end up sending the same teacher evaluation to multiple schools. Would it look bad/informal if I make copies? Or should I have my teacher make multiple drafts of the evaluation?

There is a part for a recommendation on the common app evaluation form. It says the teacher can either write directly on or attach a separate sheet–the latter is better. And it’s totally fine to make copies of one recommendation for multiple schools.

Just make sure the recommendation isn’t specified for one school, i.e. “I think X would be a great addition to the Notre Dame family.” If that’s the case, just ask your teacher to edit a bit.

Well, I was talking about copying the evaluations, which are hand-written. I know it’s ok to make copies of the typed recs, most people do that. But thanks for the replies, I’ll have my teacher type a recommendation with the evaluation.
