<p>Are we supposed to ask our teachers to fill out the Teacher Evaluation form from Common App AND write a letter of recommendation? Are they supposed to send both of them in?</p>

<p>The teacher evaluation form includes a place for teachers to “elaborate” about you. This is where they include the traditionally letter of rec.</p>
