Teacher Recommendation Mishap...super confused

Hey guys! SORRY I know I have been posting a lot lately but that's because a lot of things have been unclear and I still haven't found answers to them even now. So basically, here is my problem.

My Teacher A recommendation was sent out with a blank form and WITHOUT my signature saying that I would not seek the information blah blah blah even though I meant to sign it before it is sent....
My Teacher B recommendation was sent out WITHOUT a form at all, because the teacher no longer teaches here and I couldn't get the form to her and she sent the letter to the school and the school mailed the letter without me knowing. You get the picture.
My Second School Report was sent without my signature nor the form...because my guidance counselor insists that that's unnecessary even though I TOLD her that Columbia prefers the form lallaallaa. So that was sent WITHOUT my knowledge.....

At least it's consistent? It is a miniscule detail I know but I really don't want it to impact my admission since my chances are decent. Also, I was going to write a letter to the admissions office (almost like "additional information") to explain the situation, but my guidance counselor was saying how it's a bad idea so...now I don't know what to do because I still think I should write a letter and sign the letter with my name saying that I would not look for any of the information in any of the recommendations....

So...who do I listen to?

Bumppp REALLY need an answer

I’m pretty sure you need all those forms… I would think if you don’t have them you might even be disqualified for admission, because those forms are really important for putting all the recommendations in an even light. I would try to get them to your guidance/teachers ASAP and sent in before the deadline, even though it will be separate from the rest of your recommendation, or you should be prepared that your application will not even be considered

Uhhhhh seriously? My goodness. I hate my life.

Second opinion please. I am also calling the school for this. This is ridiuclous.

Call the school before you do anything more. This is not the kind of thing folks here can give advice on, you need to know from the admissions folks what to do next.
