Teacher Recommendations and the Common App

I just looked at the Common App for the first time and realized that the teacher recommendation includes a letter and a questionnaire about the student. I had previously thought only a letter was required. The point of this post is that my math teacher last year has written for me what I believe is a very good letter. But she is now retired and it will be very difficult for me to contact her. So, should I just forget about her letter now or can I send the letter to colleges as one of my two recommendations without the form from the common app?

I was wondering about that too. It’s not clear whether just a letter will be sufficient, or if the letter must also be accompanied by the Common App teacher form. Anyone know the answer to this?

The letter should accompany the form as it is an important component of the application. However, I’d say contact the colleges to see if they would make an exception–but at the very least, get two solid recs from teachers you are still in contact with. You may be able to use this letter as a supplementary rec if needed.
