<p>I have some questions regarding multiple schools requiring teacher recs. Say you're applying to 4 schools under the common app and to another school that requires teacher recs. Does the teacher have to fill out 5 separate evaluation forms? Do teachers have to submit an actual formal letter with the eval forms? Can teachers submit actual letters without the eval forms?</p>
<p>The teachers don't have to "fill out" the separate common app form; they can just photocopy it and only have to sign them. For the fifth, they would probably fill that one out.</p>
<p>They can either put their comments about you on the form, or write a separate letter and just put "See attached."</p>
<p>I called several of the schools my d is interested in, and all said that a letter of recommendation, without the Teacher Evaulation Form, is perfectly acceptable as long as it contains the student's full name (so it can find its way to the correct file) and what they taught the student and when.</p>
