<p>i guess your teacher can submit her own recommendation instead of answering the questions for the common app. Look on the front page of the Teacher Evaluation, where it says 'to the teacher' :</p>
<p>"....A photocopy of this reference form, or another reference you may have prepared on behalf of this student, is acceptable...."</p>
<p>But they should still send the form along with it with your info as well as their contact info and signature.</p>
