If you want to do three teacher references, which form do you use? Because usually they only have “Teacher Reference 1” and “Teacher Ref 2”. Not all the websites have “Optional Reference”–the commonapp, says it “requires 2”, but it doesn’t mention anything about submitting more than 2.

The third teacher will just have to write an individualized letter. That's usually better anyway - more personalized.

I didn't have a 3rd teacher write one for me, but I did submit a supplemental letter. All I did was submit the letter itself. Make sure your supplemental teacher/other person rec leaves contact info.

So, you just give the teacher a stamped envelope, and have them send it? No forms to attach?

Yup, just make sure that your indentification appears somewhere on the letter.

I made 2 copies of the "math/science teacher" form, without designating which one was an extra rec. Then there was also a single "humanities teacher" form.

if you are sending attitional recs, be careful. Some college stronly advise against it, although some encourage it. Make sure you check the policy of the school
