The teacher evaluation form

<p>There's a teacher evaluation 1 & 2 in the Common application. I don't see a difference in either of them. Is there a difference?</p>

<p>Is that the only sheet a teacher needs to fill out for a reference, or is a letter of recommendation needed also? I noticed it needs to be mailed to the teacher, and submitted directly to the college. What if I'm applying at a bunch of schools. Can I just have the teacher mail it to me, and then I make copies and mail those?</p>

<p>Most colleges want recommendation letters from two teachers, so you give one to one teacher and the other to the other teacher. But they should both look the same.</p>

<p>Your teachers should fill in the bubbles and attach a letter of recommendation. Both of those go into an envelope for each of the colleges.</p>

<p>I'm not sure what you mean by 'mailed to the teacher.' You should go up to the teacher and ask if he/she would write you a recommendation, and if he/she says yes, then you give your teacher the form. It does need to be submitted directly to the college, so make sure you give them stamped and addressed envelopes. Your teacher might not want to let you see the recommendation. Your teacher should make copies of forms and letters of recommendation for each of the schools and then put those in each of the envelopes, seal them, and then mail them (because you've already addressed and stamped them). It's as simple as that.</p>
