Transferring out of NYU with a low GPA, what should my next step be?

I just completed my sophomore year at NYU with 60+ credits and a 2.6 GPA. I did terribly during my freshman year due to recurring depressive episodes. I took a gap semester, came back and did well the first semester of my sophomore year, but performed poorly again the second semester (I am experiencing depression again).

I don’t see myself getting any better here. I still feel like I don’t fit in, and I barely have any friends here. But most importantly, I realized I want to change my major and focus on design and art (I usually do graphic design as a hobby during my free time)

I am aiming to transfer to a public university in New York City that offers art program, such as FIT and CUNY City College. But I am concerned that my low GPA will result in my application to be not even considered. If I have a solid portfolio, how will my low GPA affect the final decision. Do schools automatically filter out applications with low GPA?

I will still apply and give it a shot. But just in case, I get rejected from FIT or CUNY City College, what would by best next step be? Should I attend community college to raise my GPA? Or should I spend more time polishing my portfolio and apply for the next term again?
