Mail only the original application form, fees and personal
statement to the UC Undergraduate Application Processing Service.
Do not include letters of recommendation,
transcripts, test
score reports or other supporting documentation such as awards,
photographs, poetry, etc. They will not be forwarded, returned or
retained. (Supporting documentation, such as letters of recommendation,
is required for some campus scholarships and should
be submitted directly to the campus scholarship office. Refer to
pages 28–35 for more information.)</p>
