Unique situation!

I'm currently a sophomore in college and am looking to transfer for next year. My high school records do not reflect my academic abilities AT ALL (I dealt with medical issues throughout school), but I've done well in college so far.

Here are my stats:
HIGH SCHOOL
GPA: 2.6
SAT: 1100 (old version; 560 on verbal and 540 on math)
ACT: 22
EXTRACURRICULARS: Destination Imagination, a creative, team-building, problem-solving organization. I was involved in this for ten years and was awarded the Spirit Award my senior year.

COLLEGE
GPA: 3.60, but that will be raised up to a 3.70 by the end of the semester
AWARDS: Alpha Lambda Delta, psychology recognition award
EXTRACURRICULARS: Vice president of my dorm, member of my class council, mentor twice (last semester and this year) at local schools
WORK: Office on campus

I'm applying to EMERSON, FORDHAM, and LOYOLA.
What do you think my chances are for acceptance (taking into account my lousy high school record)? Don't be shy!! :]
