Universities for the average student

My junior year is almost over, I only have my final exams left. From my freshmen and sophomore year, my GPA is a 3.75. My GPA this year is most likely going to be around the 3.5-6 range. So I don't know what exactly it will be after my junior year...
This year I took AP U.S History, and for my senior year I'm taking AP Psych, AP World History, AP English, and AP Bio... my ACT is really average, I scored a 21, but I'm retaking the ACT and aiming for 25, hopefully higher.
I'm in Spanish honor society, track and field, BETA(national society that recognizes students with above 3.5 GPA), and have about 60 community service hours, but working on more.
What are some universities that would be suitable for me to apply? I would ideally like to be the Northeast or West, and I want to major in Physical Therapy.
Thank you!

not in the northeast but in a great city

Have you asked your parents how much they can afford? What is your home state?
