Urgent, please help: Where can I find a secondary school report form pdf for my 2nd counselor?

<p>Hi everyone,</p>

<p>I attended two high schools. My current counselor will have to submit my grades and school reports for both schools since I cannot assign two counselors (my current one and the one from my 2nd school).
I am having trouble finding a pdf version of the secondary school report for my 2nd counselor. Does somebody know where I can find it?
My 2nd counselor would fill out the school report for her school and send it to my current counselor. He would then upload it along with my transcript and the school report for my current school.
I have contacted the common app office twice and I have not heard from them in over a week.
Please - this is pretty urgent as I assume my 2nd counselor will need some time to fill it out and send it back!</p>

<p>The SSR is your personal evaluation. For your previous school you would have to get that from the school/counselor.
