USC requires a Counselor/Teacher report, along with a letter of recommendation.

The site also states that these can be sent separately.

Who write the letter of recommendation? Must it be filled out by the same person who wrote the report, or can I choose a different teacher?

Is the letter of recommendation a regular, generalized letter with the school’s letter head, etc.?

What kind of envelope should I put the forms in (9x12, etc.) and how should I label the envelope? Not the basic items, but should I write RE: Teacher Recommendation/Report, etc., or what should I put on the envelope so that USC knows that it belongs in my file?

my s had different people doing the report and recommendation letters. Make sure that your USC id number is on all of them. Isn't the report a form that the teachers fill out? My s had 2 teachers fill out 2 forms and 2 teachers do recommendation letters.

The letter of recommendation is however the person you asked to write it want to do. My s's teachers wrote them and put them in sealed envelopes and we put USC id number on the back of the envelopes. You can also give the number to the teachers to put onto the letter. Have you gotten the stickers with your USC id number on it? You can get the teachers to stick them onto the letters themselves

bmom - Does that mean your son had four different teachers fill the forms out? Or two different teachers filled two forms (the Report + Recommendation Letter) each?

No, I did not receive the USC ID stickers yet. I am going to call the admissions office tomorrow and ask them how to get the stickers, but from what I heard, they tell you to write your name/birthdate or SSN in the box instead.

have you applied the first part of the application yet? They send you the stickers after you applied for that. my s didn't do part 1 so he emailed admission and they gave him his id number over the phone or by email.

he had 2 reports filled out, one from each teacher and 2 recommendation letters, one from each teacher.

I just read the info and I understand your confusion. It seems to imply that you should get the recommendation letters from the same people who did the report. I can't remember whether it was phrased that way in my son's app or not. The best thing for you to do is to send admission an email and ask them to clarify. They're very good at answering questions. On their website, under undergrad admission, under contact us, choose admission.

If I remember correctly there is a form for the GC to fill out as well as a form for each of two teachers.

On the GC form there are a number of boxes he/she needs to check off as well as a space for the GC to add comments. GC can also enclose a separate letter if wished. At my D's school the GC also enclosed a school profile in the envelope. If your school has a profile, your GC will most probably automatically enclose it. (The profile is a description of your school, grading procedures, etc, info that allows the college to know more about your school)

These three forms can be downloaded from the site and given to the GC and each teacher. (I don't remember if its the same form for teachers that you download twice, or whether they are indicated as "Teacher1" and "Teacher 2") Teachers also fill out this form, checking boxes etc. and include their rec letters in the envelope.

It's best to send forms and letters in the larger envelopes so as not to have to fold the papers.
The address where to send I believe was indicated on the forms. If not addresses are listed on USC website.
On the outside of the envelope, in one corner, indicate the contents and your name, ID# if you have it.

At my D's school, students were instructed to give the forms along with stamped addressed envelopes to GC/teachers (with your school's return address), and the school itself sent everything to the college.

My D never used the bar code stickers since she received them late.
You can definitely put down your SSN/name, birthdate or your USC ID# (if you decide to do the Part 1, after which they will send you the ID #))

USC is very flexible and aware that applicants don't always submit stuff as per instructions. It can be confusing when you start all the steps. They are also very helpful if you need to call them.

Thank you very much - honestly I'm still a little slightly confused even after calling the USC Undergrad Admissions Office and the replies - but I will keep asking and post the definite answer here!

Did you talk to your guidance counselor at your high school? If you are from California, your guidance counselor should be very familiar with admissions policy at USC. Also, in the fall, USC sends representatives to individual high schools and to regional events. I suggest you try to meet the USC rep when s/he comes to your high school and attend the USC regional event (see "USC on the Road"---USC web site for dates). My son's high school had very specific guidelines to follow for recommendation letters, how many stamps on the envelope, size of envelope, etc. If your guidance counselor can't help you, speak to the head of guidance counseling and/or your college guidance counselor at your high school. It is important to put down your USC ID# and your name in the correct places on the recommendation page(s). Also, last year, USC provided recommendation forms in its admissions packet brochure. Did you look for the recommendation forms to provide to those you want to write recommendations?

hey I'm also applying for USC too. I just got my USC ID number in the mail. However, I have no idea where to get the teacher rec forms. I tried searching online and I can't find it anywhere. Can someone give me the link please? thanks
