<p>The USC site says to have a Counselor/Teacher report filled out along with a letter of recommendation.</p>
<p>1) Can the report and letter of recommendation filled out by two different people</p>
<p>2) Is the letter of recommendation a regular letter with the school header, etc? What exactly should I request the teachers write in the letter of recommendation?</p>
<p>3) When sending the letter of recommendation to USC, how can they identify the letter of recommendation to mine? Or how can I format the letter of recommendation correctly so that it is filed correctly?</p>

1) Online fee waivers - how do I obtain this? I understand my guidance counselor has to request one for me, but which form do I use? NACAC? Or...</p>
