I currently attend the University of California of Irvine majoring in Biological Sciences, and I plan on transferring out to enroll at USC as a Kinesiology major or a Human Biology Major. I currently have a 3.7 GPA and I will be starting my 2nd year at UCI (but I am classified as a Junior status, making me eligible for a transfer by February 2014). Unfortunately, I came to decision to want transfer during my Spring Quarter as it may be considered of a late notice. In addition, for only being a 1st year here at UCI, I feel as I may not have much on my application to be a stunning applicant.

I have several questions:
1. For the USC Transfer Application, it says that there needs to be a teacher evaluation as a supplement for the application. Am I allowed to submit more than one teacher evaluation or only one?
2. Sadly, I am not involved in many campus activities that make me well-known among my peers, like being in a club, fraternity, or even intramural sports. However, I do work on campus with a professor in his lab and volunteering with his research, I work at a doctor's office as a receptionist and office assistant, and I intern at a hospital as a Clinical Care Extender. Would it be wise for me to seek campus activities this incoming school year? I feel that this may be straining my workload and possibly risking my GPA as a transfer.
3. What do you think my chances of making it into USC as a Kinesiology or a Human Biology major, assuming I keep a minimum of 3.7 GPA and my current workload. I will also be including information from high school on my application as being the Distinguished Club President of Key Club International of all of California, Nevada, and Hawaii, being the varsity doubles tennis team captain who was MVP during my season, Doubles Champion, and also placed 1st Team All League Doubles and 2nd Team All League Singles. I was also a part of my high school's National Honor Society, California Scholarship Foundation, and a club officer of a leadership group. I was awarded the U.S. Army's National Scholar Athlete Award.
4. I'm a little confused on the transfer packet. What exactly do I need to turn in as a transfer applicant?

Thank you for you help and I greatly appreciate it.
