Very weak high school transcript, much stronger college transcript, transfer to a top 20 school

I had a 77 GPA in high school with only two APs.
I currently attend a city college and I got a 3.8 first semester with 4 classes, 13 credits.
I'm taking 5 classes this semester, and I hope to do just as well.
2050 on my SAT, 29 on the ACT, will probably send the SAT.
I have decent extra curriculars, I believe. I have some volunteering, I'm involved in my community, I have a good story.
March 15 is the application deadline for Vanderbilt University, (my reach). I will also be applying to a few SUNYs, Boston University, Rutgers, NYU-Poly, and a few others.
Realistically speaking, should I bunker down in my college for a bit longer so my second term grades come in or should I pull the trigger and go for the fall semester?

if you want to maximize your chances of admittance you should apply for Junior level standing. I.e. 60 transferable units completed before admission, a declared major with pre-reqs completed and the GEs (find guides for the universities you are apply to) knocked out too.

Vanderbilt lists their general education requirement here.
<a href=“AXLE | academics | undergraduates | Internal | College of Arts and Science | Vanderbilt University”>http://as.vanderbilt.edu/academics/axle/

One word of caution is some courses may transfer over as credit but not satisfy the requirements. E.g. Most CC philosophy courses would not have enough writing to satisfy the writing requirement.
