<p>When I filled out my FAFSA, I got a message saying that the Social Security Administration didn't confirm that I was a citizen. So, I sorted this out with the office by showing them my passport, however I still have to show proof to the schools I applied to (which I did by fax). But it still says on the college portals that I haven't sent it in. I figured it would be strange just faxing a passport unaccompanied by some type of sheet. So my question is this: is there some type of cover sheet I have to send along with the passport or what? If it helps the schools I need this for are Duke and Cornell.</p>
<p>You can create a simple cover sheet or find a template in most word processing programs. You should put the name of the person you are faxing the information to, your name and if you have an ID number with them include that as well, and a very brief explanation of what you are sending and why. </p>
<p>It sounds like you already faxed your proof and it just hasn’t shown up at some schools. It could just be a delay in processing / entering the update into the computer system. You could try calling and asking if you should re-submit. Or just re-submit and then email stating what you have done and asking them to please confirm receipt.</p>
<p>You also could email them, and ask if you can scan the passport and email that scan to them as an attachment. If you or one of your pals have a scanner at home, that might be the quickest way to go.</p>
