<p>I find this a little bit tricky.
I'm using common appliation now. My understanding is that, Midterm report is just a report of letter grades for the classes I am taking. So ideally, I print it out, go to each professors of those classes that I'm taking right now, request them to write down the grade and sign on it, then send it to the school.</p>
<p>But what if I waive my right to acess? Do I just give it to my advisor and she will take care of it? or Do I print a midterm report for each professor (5 mid-term reports in total)?</p>
<p>You ignore the waiving of rights for the mid-term grade report. Print out one copy, take it to all of your teachers to sign and then send it in.</p>

<p>I think a lot of people are confused about the wording of the forms. It says nothing about you not being able to see it. It only says that after you submit it, you won’t ask to see it again.</p>
<p>“I understand I will never see this form or any other recommendations submitted by me or on my behalf.” - if it was submitted by you, you’ve seen it.</p>
<p>In many instances, arcadefire, the person filling out the form will hand it back to you already sealed (or just mail it for you). You are waiving your right to ever see it, which encourages people who are writing stuff about you to be honest. For example, I have no idea what my evaluation from my transfer application said, and I will never see it.</p>
<p>That said, this isn’t really a subjective form, so it shouldn’t matter whether he sees it or not.</p>
