What all should I give someone writing a letter of recommendation?

For my English teacher, who I had sophomore year, I am including a resume, an explanation of all of my extra-curricular activities, a copy of an essay from the year I had her, a copy of an essay from junior year, a stamped and addressed envelope to the school for which she is recommending me, and a copy of the teacher evaluation sheet.

<p>That sounds like more than enough to me, but the best way to find out is to ask the teacher. Some teachers have different preferences.</p>
