What are my chances for the UCs?

SAT Scores: 780 English, 790 Math
PSAT Scores: 1510
GPA:
3.96 unweighted
4.42 for all three years in high school, 4.65 UC GPA (sophomore and junior year)

Extra-curricular activities:
Volunteer at the hospital every summer, more than 400 hours
Shadowed an internal medicine team
Shadowed a physician
No clubs
Volunteer in a community organization
Voices, part of the Belmont Library
Started a program to teach cooking classes at a local homeless shelter

Are you instate? OOS students pay full fees of $55k per year.
Your SAT looks good. GPA is good. Depends on the major.

The biological sciences major is impacted, so you have to hope on having a nice package with good essays.

Would it be better to apply undeclared or do pre med with something else as a major?

They don’t have a premed major. Applying undeclared could be risky.

Pretty sure you’ll easily get into all of the UCs.

Biology is an impacted major at a few of the UC’s, so if you want Biology, then apply as a Biology major. If the major is impacted or capped, going in as an Undecided may make it difficult to switch into the major later since it could require a specific GPA in pre-req courses before making the change. Going for the direct admit into the major is your best option.
