<p>If you scroll down to the part II of the application (step #2), you will see a list of forms to download. I read somewhere that although it may seem redundant, you are encouraged to send in form 5 even with the college score report.</p>

<p>Do you have to submit form 5 even if your scores are already on your transcript?</p>
<p>Last year my daughter had to fax/send in her SAT scores to USC despite the fact that her counselor said she was going to put in her recommendation and the collegeboard also send in her SAT scores separately. I think it helps USC admission to make decision faster.</p>
