Should I ask the professor to print it out and hand it to me in a sealed envelope, ask him/her to mail it directly to the school? Give him/her the common app evaluation form?

i would ask the professor to print it out and hand it to me in a concealed envelope. I wouldn’t risk letting the professor send it. They may forget or send it in late. Being a professor isn’t easy, especially one with a family. It is always safe to hand something in yourself.

You need to print out the Instructor Evaluation form because you need to fill out the top part. Then give it to your Instructor to fill out the rest. You need to give them an envelope and they should sign across the flap. Whether you want to address and put postage, or have them return it and mail yourself is up to you. Either way, I would recommend using your address as the return address, not the instructor’s.

What do I do if I’m applying to more than one college on the common app? Make him/her fill out the form for every college?

Yes, they need to be hardcopy, so that’s the only way to do it. It shouldn’t be that hard. If you give them the Instructor form with the top filled in, they can fill out the rest except for the signature and make copies. They can then sign the copies of the form and attach copies of the LOR if necessary.

Actually, all you need to do is enter their name and email into the Common App, which then emails them and eventually tells them how (password, presumably) to upload their recommendation and to fill out the evaluation form. Then it’s online and you can use it for as many schools as you wish, all electronically. Unless a certain school says hardcopy necessary, go the easy way.

^Unless the CA has changed this year, there is no “easy way” for transfer applicants. I’m not on the CA, so transfer people, is this a new option?

Hadn’t realized this was transfer. I imagine you’re right. My only experience is with high school.

^Please note the forum.

Just saw title in new posts and didn’t see forum. Don’t worry, I won’t be back.

^Now your post about Barnard on the Y forum makes sense…;)
