What should a teacher put on the letter of recommendation

I am a teacher and this year was my first year of being asked to do letters of recommendation. I was wondering if anyone could tell me if there was something significant that they felt helped them stick out amongst their peers.

I know the letters are supposed to be from me, and usually I write about how the student did in school, how they behaved in class/personality, any EC's I may know of.

If anyone can knows of other stuff that can be included please let me know.

And no, I am not thinking of writing this for my son, and no, I do not want to lie on the letters.
