I did some research in a lab this summer. Originally I had not planned to send a rec from my mentor, since i didn't think it would make that much of a difference. But after some discussion with friends/parents they all agree that it wouldn't hurt and it would be helpful if anything.

However, I am not sure how I should have my mentor send in a rec. Should I have him fill out the teacher evaluation form on common apps? I am leaning towards just having him send in a letter since I know how busy he is. But would it be a problem for colleges to match it up with the rest of my application? (Btw, i did my research at Texas Tech as part of the Clarks scholars program and i live in ohio, so I am planning on having him send them directly to the colleges)

Also, I already have had 3 teacher send recs. (one english, calculus, and my health teacher who mentored me on a community service project) is sending another excessive?
