<p>Like how will they know how to file it? Should the person include something like a "intro letter" saying what the letter is and identification info? How much identification info should be put on? (Like if we don't want to give out our SSN just in case)</p>
<p>So you’re asking if the author of your letter of recommendation should write a letter introducing his or her letter? </p>
<p>No. If you are using an instructor evaluation form, use that and give it to the person with a stamped enveloped. If you are not doing the Common App or the person is not an instructor then they should just write the letter being sure to use your full name. They should send it directly. </p>
<p>This is something you really don’t need to be worried about.</p>
<p>Have them put your full name as you put it on the application, your birth date, and “Supplemental Recommendation” (so that it’s not mistaken for a primary LOR) on the letter.</p>
