Where do I apply? Please help!

I'm in community college currently and don't know where I have a realistic shot of attending. My situation is kind of a unique one (not many are in my situation that I know of) so I'll get into that a bit too.

I graduated from high school in 2007 and matriculated at University of Michigan, Ann Arbor. I was dealing with personal issues, depression, ended up abusing certain substances to avoid dealing with problems, etc. I ended up averaging less than 2.0 in 3 semesters there, was dismissed from the school.

I have been attending Nassau Community College (in NY) full-time while working 20-25 hours per week. I've had to work my way through school and even worked 25-30 hours while attending school full-time at Michigan. I still work about 25 hours a week while attending community college full-time.

general stats

~3.8 weighted GPA in high school
sub-2.0 at UMich (about 30 credits)
4.0 GPA currently in community college (approx. 35 credits, with 15 in progress)

2180 on SAT I (750 verbal, 730 math, 700 writing) taken in 2006-07

Variety of EC's like JV/Varsity high school sports, honor society, music honor society, numerous volunteer positions.

I have a substantial amount of professional experience working as a web/flash designer. Have worked steadily with one of the largest healthcare web design companies in the U.S. for nearly 7 years on a part-time basis (full-time over summers), starting my in junior year of high school. I've had numerous free lance website projects, volunteered as a web designer with a number of non-profits, internships with reputable companies (e.g. Merill Lynch).

I applied to a few schools for this current semester Spring '12. Mostly top-25 schools and I was rejected to all of them. The one school I did get into was SUNY Stony Brook but I wanted to see if I could get into a more reputable institution with an additional semester of coursework under my belt. I really want to show schools I'm serious about my studies and that I am making a full 180 degree turn from my time at UMich.

I want to make the March 1st (for most schools) deadline for the Fall'12 semester and really have no idea of where I have a realistic chance of getting into. Any ideas, suggestions, or advice is truly appreciated. I want to go pre-med and so a school with a good reputation for sending kids to med school or a good rep for quality science courses would work too.

I am from the east coast as well, but am willing to apply pretty much anywhere.

Sorry, I forgot to include some prospective schools.

Basically, would I have a realistic shot at a top-50 university or a top-25 liberal arts school?

Sorry to double post but am not sure how to edit? I am new to the forums and am still trying to get used to everything.

The schools that I’m certain I’ll be rejected to but want to apply anyway because of the one in a million shot is Duke, Johns Hopkins, and Washington University in St. Louis primarily for their pre-med reputation (as well as their all around academic reputation).

A few top-50 universities I plan to apply to are UNC Chapel Hill, College of William and Mary, Georgetown, U of Florida, U of Miami, U of Pittsburgh, U of Connecticut, and Syracuse University.

A few top-20 liberal arts schools I plan to apply to are Williams, Swarthmore, Pomona, Claremont McKenna, Davidson, Haverford, and Wesleyan.

If you might have recommendations for schools I might consider for pre-med (or any other reason) I’d appreciate the advice.

Thanks in advance. I am very unclear on what my chances of getting into any of those schools are given my unusual academic circumstances and am truly appreciative of any advice you might be able to provide.
