Where is the school report form?

<p>My counselor has opted out of doing these electronically, so she has requested that we submit a blank copy of the form and she will fill it out. However, when i go to the Common App section for it, it only says:
Counselor
You must invite a school Counselor who will complete the School Report and other forms for you.</p>

<p>Invite Counselor</p>

<p>In other words, it says that I can only invite a counselor who wants to do it by hand.
Then when I look up the form, the link outputs an error.
Does anyone have a working link or want to send me a copy of the School Report Form, or have any other relevant advice?

<p>You do still need to invite the counselor electronically through the common app by inputting the counselor name and email address. If the counselor has already opted out of online submission, your app will automatically update to show you the printable pdf forms.</p>

<p>Invite the counselor, but leave the email address field blank (note that the email address field is not required). When you submit the invitation, you’ll see a link that provides access to a printable form you can give to your counselor.</p>
