I know it states that a dean or academic advisor is supposed to do it, but the only person I've been able to contact who can help me is some sort of manager at my school's registrar (I think he typically fills out these sorts of forms and also has access to my academic records). Do you think this would fly?

If that’s who does it at your school, you’ll have to go with it. They do put the title of their position on the form, and you might also ask them to write in the evaluation section that they are the designated person to fill out the form, but that they do not have the capability to fill out the Rating and Evaluation sections.

I think “Manager of the Office of the Registrar” sounds pretty legitimate. I don’t know this guy on a personal level to any degree (I’ve only spoken with him on the phone), but I was planning on providing him with a small list of the extracurricular projects I’ve been working on at this school so he’d have SOMETHING to say in the rating and evaluations sections.
