who still has not sent in medical information documents??

<p>Am I missing something? I only have one sheet (with two carbon copy papers underneath) entitled the "Student Health Plan Waiver/Enrollment Form." It's all about insurance plan; there's nothing requiring a doctor's signature or any mention of shots or vaccinations. What are you guys talking about?</p>

<p>ware_ru, you should search for the "Medical Information Form for Incoming students". It looks similar to that enrollment form, but it has 2 pages.
I always thought I am the lazyest of all the people at Princeton :( ... It turns out I'm not :) But I'm close :)</p>

<p>I'm sending mine out tomorrow. I had to get three shots, an x-ray and a blood test for this form rawrrr </p>

<p>Sucharita, you don't need to send the x-ray but you have to include the written statement that you were negative.</p>

<p>wait.. so we dont have to pay a fine for lateness if we get the shot form thingy in before August 30th (mentioned in previous post) even though the paper says "get this beast in by June 30 or else we hope you dont like your first born" ? just need to clarify b/c i was in the process of moving and am just now finally able to get in someplace to get whatever immunizations I lack.</p>

<p>hi i actually just called their office and spoke to them regarding the June 30th deadline. My doctors have been on vacation for two weeks (convenient) and were unable to administer the mantoux skin test until today. The school med office said it would be fine as long as i just got it in before august...no fine. ;)</p>
